Survey Instructions: Please read the following directions then click this link - I am ready to take the Profile Survey

  1. There are 40 Survey Items. In each Survey Item, order all 5 Endings from Most like me (at the top of the list) to Least like me(at the bottom of the list). You do this by using your mouse to click on the Ending you want to move, and then clicking and dragging it to the location you want.
  2. Continue moving each Ending until you have the order you want.
  3. Please take the complete survey without interruption.
  4. You will notice repetition in the Endings. This is part of the nature of the survey.
  5. There are no right or wrong choices in this survey. All ways of answering this survey are good.
  6. After you are satisfied with the order of the a Survey Item, use the "next survey item" button to go to the next Survey Item. Continue this process until you have ordered the Endings in all 40 Survey Items. When you are satisfied with your choices for all 40 Survey Items, press the "Submit" button.
